WAIVER FOR SERVICES

Below is our General Liability Waiver. This must be signed by all clients receiving a service from The DogSpaw LLC. To sign your waiver, you can do so on your Pawfinity account or when you come in for your next service.

GENERAL LIABILITY

The DogSpaw employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects of the grooming process, including but not limited to: razor burn, exposure of the quick, reactions to flea treatments and de-shedding treatments, hair splinters, and sharp nails (after clipping). By signing this agreement, you (the client) agree to relieve The DogSpaw, and it's agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by The DogSpaw. The pursuit of any veterinary, behavioral, or rehabilitation services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve The DogSpaw and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by the DogSpaw and it's agents.

CANCELLATIONS AND NO SHOW FEES

If you cancel your pet's appointment within 24 hours of the appointment or do not show for your scheduled appointment, there is a $25 fee that must be paid prior to rescheduling your pet's appointment. This fee is per pet, not per household.

TARDINESS

We can no longer accept any pet that arrives 15 minutes past their scheduled appointment time. Services will be declined after the 15 minute arrival time and a $25 rescheduling fee will apply, per pet. This fee must be paid prior to rescheduling your pet(s).

COMMUNICATION

We do require an active e-mail address for all clients as well as a current phone number. Your e-mail will be used to send updates in case of severe weather, invoices for missed appointments, unexpected closures, etc.

MATTED PET RELEASE

Severe matting on your pet may cause his/her skin to be irritated for a few days. This irritation may not appear for 2-3 days after grooming. We have taken every precaution to minimize the possibility of this happening. In some cases a change in personality can also happen. Hiding, depression, pouting are examples of this change. This is temporary and will generally resolve itself in 2-3 days.

Your groomer has determined that there are mats that need to be shaved in order to remove them safely and painlessly on your pet. Signing this release not only gives your groomer permission to shave your pet, but also gives The DogSpaw permission to seek veterinary attention in emergency circumstances if you cannot be reached.

You will be provided an information sheet on best practices to prevent tangles and matting for your pet. Please read it thoroughly.

By signing this agreement, you grant permission to authorize emergency medical care for your pet(s) as deemed necessary by a veterinarian, and you will be responsible for full payment of such care. The DogSpaw will always call you first. If you are unable to be reached, you give us permission to seek veterinary care for your pet and make medical decision based on the best interest of the pet in cases of an emergency.


MAINTENANCE FEE/HANDLING FEES

Pets that require muzzling, additional brushing, dematting, or extra staff to help manage them will be charged a maintenance fee or handling fee in addition to the normal grooming rate. This is because pets showing aggression, acute nervousness, and/or poor coat conditions take much longer to groom.

This rate will depend on the amount of time and assistance required to groom your pet. There may be times that the groomer will not be able to give you a precise fee amount until the groom has been completed.

By signing this agreement, you agree to any additional maintenance and/or handling fees associated with grooming your pet.

HEALTH ISSUES

If your pet has any allergies or medical conditions (epilepsy/seizures, diabetes, arthritis, asthma, etc.) it is extremely important that you make The DogSpaw aware of any medical or behavioral issues so that The DogSpaw can take the necessary steps to ensure your pet's safety and comfort.

MEDICATIONS

There are certain medications that are prescribed by a veterinarian for anxiety and/or behavioral issues. You must inform The DogSpaw PRIOR to the appointment to make sure it is a medication that does not require medical monitoring. Some sedatives are too strong and require monitoring by a medical professional to ensure the safety of the pet.

REFUSAL OF SERVICE

The DogSpaw has the right to refuse any services at any time. In the event that your pet is too stressed or becomes dangerous to groom, or becomes a danger to staff, The DogSpaw reserves the right to refuse any services, stop services and/or cancel services at any time.
